PAYMENT POLICY: Payments made more than 7 days after the due date will be considered late, and a $20.00 late fee will be charged in addition to the month’s tuition. This applies to LA Judo and/or Eplex Sports Academy.

 

ATTENDANCE: The student acknowledges that class attendance is the student’s responsibility and is crucial for the student’s progression in rank. Failure to attend or complete classes does not relieve the student of the obligation to pay tuition in full for LA Judo and/or Eplex Sports Academy.

 

REFUND & WITHDRAWAL POLICY: All tuition and registration fees are non-refundable. Any withdrawals from enrollment require two weeks' notice before the intended date of withdrawal. This policy applies to LA Judo and/or Eplex Sports Academy.

 

PARKING LOT SAFETY REGULATIONS: LA Judo and/or Eplex Sports Academy are not responsible for theft or damage to personal property such as vehicles or valuables.

 

ADVERTISING CONSENT: I hereby give my consent to LA Judo and/or Eplex Sports Academy to use photographs of my child for commercial or promotional purposes.

 

SPECIFICATION: I understand that LA Judo and/or Eplex Sports Academy are private institutions and are not under the jurisdiction of the Community Care Licensing Department.

 

WAIVER OF LIABILITY: In consideration of being granted permission to utilize the programs, attend classes, observe classes, and access equipment, property, and facilities at LA Judo and/or Eplex Sports Academy, I, on behalf of myself, the student, personal representative, or assigns, hereby release, waive, discharge, and covenant not to sue LA Judo and/or Eplex Sports Academy, its directors, officers, employees, and independent contractors from any and all liability arising from claims, including those arising from the negligence of LA Judo and/or Eplex Sports Academy, related to personal injury, accidents, illnesses, and property loss. This release encompasses incidents arising from, but not limited to, participation in classes, activities, observations, and the use of facilities, premises, or equipment.

 

SEVERABILITY: The foregoing waiver, assumption of risks agreement, and indemnity agreement are designed to be broad and inclusive to the extent permitted by the law of the State of California. If any portion thereof is deemed invalid, it is mutually agreed that the remainder shall remain in full legal force and effect. I have thoroughly reviewed this waiver of liability, assumption of liability, and indemnity agreement and fully comprehended its implications. I acknowledge that by signing this agreement, I am relinquishing certain rights, including the right to pursue legal action. I am affixing my signature to this document of my own free will and voluntarily. It is my express intention, through this signature, to affect a complete release of all liabilities to the greatest extent allowed by law.
